VERIFICATION
Tell us when a document and its record do not agree.
A wrong date, unexpected version or altered-looking page deserves a careful review. This route lets recipients raise a concern about a document presented as issued by FAFA. A concern is recorded for assessment; it is not an automatic finding of misconduct or fraud.
Tell us about the document
Describe the discrepancy precisely
Include the reference, the source of the document and the detail that appears inconsistent. A short description may be enough to begin. Do not send the complete confidential file through an unrestricted form. The team can arrange an appropriate transfer method where evidence is required.
Preserve the information you received
Keep the original file and the relevant communication without editing them. Avoid confronting other parties based solely on a not-found result or an unfamiliar format. We may need to clarify the version, issue history or disclosure permissions before explaining what the register can confirm.
Respect the limits of disclosure
The firm may be unable to disclose client information to an unauthorised recipient. The review can still identify the appropriate next step or confirm permitted issue details. Any legal, regulatory or professional escalation is considered through the relevant process, not decided by an automated website label.
Common questions
Will submitting a concern give us the full report?
No. Access remains subject to authorisation, confidentiality and the purpose for which the document was issued.
